I, the undersigned, as the parent or legal guardian of the above-named minor, hereby give my consent for my child to use the tanning services provided by Dream Tanning, operated by Dream Spa Limited. I acknowledge that I have read and understood the following terms and conditions:

Awareness of Risks: I understand that the use of tanning beds involves exposure to ultraviolet (UV) radiation, which can have potential risks, including but not limited to skin damage, premature aging, and increased risk of skin cancer.

Health Conditions: I confirm that my child does not have any health conditions or is not taking any medications that may adversely affect their ability to use tanning beds. I have consulted with a healthcare provider if necessary.

Usage Guidelines: I understand that my child must follow all instructions provided by Dream Tanning staff regarding the safe and effective use of tanning beds. Failure to do so may result in injury or adverse effects.

Liability Waiver: I agree to release and hold harmless Dream Tanning, its staff, and its affiliates from any and all claims, damages, or liabilities arising from my child’s use of tanning services.

Punctuality and Cancellation Policy: I understand the importance of punctuality and the cancellation policy, including the requirement to cancel or reschedule appointments at least 3 hours in advance to avoid penalties.

本人作為上述未成年人士之父／母／合法監護人，現同意讓我的孩子使用由「Dream Spa Limited」營運的 Dream Tanning 所提供的日曬服務。本人確認已閱讀並理解下列條款及細則：

風險認知： 本人明白使用日曬床涉及暴露於紫外線 (UV) 輻射中，這可能存在潛在風險，包括但不限於皮膚損傷、過早老化和增加患皮膚癌的風險。

健康狀況： 本人確認我的孩子没有任何健康狀況，也沒有服用任何可能對其使用日曬床的能力產生不利影響的藥物。如有必要，本人已諮詢過醫護人員。

使用指引： 本人明白我的孩子必須遵守 Dream Tanning 員工提供的所有有關安全有效使用日曬床的指示。否則可能會導致受傷或不良影響。

免責聲明： 本人同意免除 Dream Tanning、其員工及其附屬公司因我的孩子使用日曬服務而產生的任何及所有索賠、損害或責任。

守時和取消政策： 本人明白守時和取消政策的重要性，包括需要至少提前 3 小時取消或重新安排預約以避免罰款。
